Includes recommended interventions, detailed evaluation report and clear explanation of evaluation findings. Formal documentation of a learning disability is also provided when applicable.
Dyslexia Evaluation

The dyslexia evaluation process at Forge Education is for families and individuals who are looking for a dyslexia specific evaluation.
While early indicators of dyslexia can be seen before kindergarten, a comprehensive evaluation is not recommended until the individual has received at least one full school year of appropriate literacy instruction. Evaluations can be administered for children, adolescents and adults.
If you are interested in assessments for other neurodevelopmental disorders or psychological conditions, such as anxiety or ADHD, you must seek a full neuropsychological evaluation.

Evaluations are extensive and include
Family/caregiver intake session(s) covering developmental and educational history, records review and current concerns
Collaborative consultations with teachers, schools, pediatricians and other providers
Evaluation sessions using a battery of standardized assessments to determine an individual’s strengths and deficits
Assessment scoring, analysis and report writing
Detailed report with assessment scores and findings
Family/caregiver feedback session to share report findings, recommendations and answer questions

Evaluations range in time from 2-4 weeks once the process has begun.
Evaluation Process
Parent/caregiver intake session (1.5 hours)
Collaborative consultation with teachers, pediatricians and other providers (1-2 hours)
In-person evaluation sessions (3-6 hours, can be split into multiple sessions across multiple days)
Report scoring, analysis and report writing (1-2 weeks)
Family feedback session (1-1.5 hours)

Even if an individual is identified as having a specific learning disability in reading or dyslexia, there is no guarantee that they will qualify to receive special education services.
With the identification of a learning disability such as dyslexia, they should qualify for accommodations through a 504.

Evaluations range in price from $1,400-$2,600.
Final cost is determined by the time required to complete the entire evaluation process.
$1,400 deposit due prior to start of evaluation process.
Final cost estimated after first evaluation session and is due prior to the Family feedback session.
